About Us We are a pioneering organization in the realm of in-home
clinical networks, dedicated to providing comprehensive health
assessments nationwide. Our mission is to deliver exceptional,
personalized care that enhances health outcomes, reduces costs, and
empowers individuals to lead fulfilling lives. With a strong
foundation in clinical assessment and care management, our
extensive network of clinicians transcends traditional barriers by
meeting members in their own environments. We focus on improving
the quality of care for older adults and at-risk individuals,
helping them achieve better health outcomes and identify chronic
conditions that may otherwise remain undetected. Our available
positions enable you to utilize your skills and compassion to make
a tangible difference in the health and well-being of others. Join
our team and enjoy competitive pay and flexible scheduling while
positively impacting the community! Responsibilities About the Role
During visits that may last up to one hour, our providers assess
and observe a member’s health status, medical history, medication
adherence, social circumstances, and other potential risks. This
approach offers invaluable insights into a member’s overall health
that are often missed during standard office visits. The Matrix
Comprehensive Health Assessment (CHA) aims to enhance care quality
and allows us to address multiple care gaps in a single visit. Our
providers are responsible for: Conducting assessments for adults
and geriatrics, including medical history, diagnosis, treatment,
health education, physician referrals, case management referrals,
follow-up, and thorough documentation in line with our guidelines.
Administering and ordering necessary point-of-care and lab-based
tests. Collaborating with physicians, case managers, social
workers, family members, key caregivers, and other medical
personnel as needed. Working alongside Primary Care Physicians
(PCP) to provide patient education and follow-up as required.
