Nurse, you will play a crucial role in providing high-quality
patient care in a home health setting. Your primary responsibility
will be to assess, plan, implement, and evaluate patient care plans
tailored to individual needs. You will collaborate with a
multidisciplinary team to ensure that patients receive
comprehensive care that promotes their health and well-being.
Additionally, you will educate patients and their families about
health management and disease prevention, empowering them to take
an active role in their care. Ultimately, your efforts will
contribute to improved patient outcomes and enhanced quality of
life for those you serve. Minimum Qualifications: Current and valid
Registered Nurse (RN) license in the state of Indiana. Bachelor of
Science in Nursing (BSN) or an Associate Degree in Nursing (ADN)
with relevant experience. Basic Life Support (BLS) certification.
Preferred Qualifications: Experience in home health nursing or
community health settings. Certification in wound care management.
Knowledge of electronic health record (EHR) systems.
Responsibilities: Conduct thorough assessments of patients' health
status and develop individualized care plans. Administer
medications and treatments as prescribed, ensuring adherence to
safety protocols. Provide direct patient care, including wound
care, telemetry monitoring, and assistance with daily living
activities. Educate patients and their families on health
management, medication usage, and lifestyle modifications. Advocate
for patients' needs and preferences, ensuring their voices are
heard in the care process. Supervise home health Aides Skills: The
required skills of home health nursing and direct patient care are
essential for delivering personalized and effective care to
patients in their homes. Patient education is a critical component,
as it enables you to empower patients and their families with the
knowledge they need to manage their health conditions. Wound care
and telemetry skills will be utilized daily to monitor and treat
patients, ensuring their safety and comfort. Patient advocacy is
vital in this role, as you will be the voice for your patients,
ensuring their needs and preferences are prioritized in their care
plans. Overall, these skills will enable you to provide
compassionate, comprehensive, and high-quality nursing care.
